• Dietary & Activity Guidelines: Specific recommendations for your diet and activity levels during recovery, including any restrictions or exercises.
• Warning Signs: Information on specific symptoms or signs to watch out for that might require immediate medical attention, and who to contact.

You will be provided with a comprehensive set of documents for your records and for your ongoing care:

• Discharge Summary: A detailed report summarizing your diagnosis, treatment received, hospital stay, and the complete post-discharge care plan.
• Prescriptions: All necessary prescriptions for medications to be continued at home.
• Follow-up Appointments: Details of any scheduled follow-up appointments with your doctor or other specialists at Miracle Hospital or elsewhere.
• Referrals: If required, referrals to home healthcare services, rehabilitation centers, or other specialized support.
